Personal Finance (Not Investing) • Medical insurance question

I had a minor surgery last year, went fine. Insurance covered it. Now I received a letter in the mail from the insurance company/subrogation company asking me to certify it was really me that had the procedure and asking questions. I would rather not bother with this. I had already put it out of my mind and don't want to think about it. Can this be ignored, or is it typical and I should respond?
The contract you have with them may require your cooperation.

Just fill it out and send it in. They're hoping they can go after someone else to recover some costs.

I got a similar letter after a mandoline incident.
