The contract you have with them may require your cooperation.I had a minor surgery last year, went fine. Insurance covered it. Now I received a letter in the mail from the insurance company/subrogation company asking me to certify it was really me that had the procedure and asking questions. I would rather not bother with this. I had already put it out of my mind and don't want to think about it. Can this be ignored, or is it typical and I should respond?
Just fill it out and send it in. They're hoping they can go after someone else to recover some costs.
I got a similar letter after a mandoline incident.
