A few years ago I got a notification from Fidelity about a QCD check they had sent to me that had never been cashed. (They had mailed me the check, and I then mailed it to the charity with a cover letter.) I called the charity; they said they never received the check, which apparently was lost in the mail. (Thanks again, Mr. DeJoy!) Fidelity put a stop payment on the check and mailed me a replacement. I appreciate their proactive approach.
I usually send out 15-20 QCD checks a year, always with a cover letter or a donation form such as the ones some charities mail me in annoying quantities throughout the year. It's a chancy process, perhaps, but there are also downsides to having the check mailed directly from the custodian to the charity. I do wish, though, that IRA charitable distributions could be made via ACH or similar digital system.
