In my state (MA) you would contact your exchange about 45~60 days before your wife goes on Medicare to update your ACA application to remove your wife.
Regarding HMOs and travel. for domestic, our HMOs would cover us as in network when traveling out f state. We were advised that some out of state providers may require upfront payment though, in which case we would have to seek reimbursement for those payments. International travel no coverage (we bought coverage through tour provider). YMMV….
PS- if you obtain dental insurance through the exchange, your wife may be able to retain it during the transition to Medicare. Our exchange allows this , and has decent dental plan options that we can use to obtain network pricing with our dentist. We both have continued our dental plan while on Medicare.
