This turned into a bit of a mess, but as of this morning I see the Vanguard balance finally zeroed out. From what I can tell, Fidelity physically mailed the paperwork the first time and nobody knows what happened to it. They attempted to mail it again for the followup, but my case manager realized it may not make it in time. He called Vanguard while I was on hold and got a fax number to resubmit the request, which Vanguard handled on their end that day.A note for those moving to Fidelity: when you fill out the transfer paperwork and select "transfer entire account", consider listing the funds anyway to make sure they all get transferred. I didn't, and they only transferred the funds listed on the months-old 401k statement (the most recent possible, since Vanguard only provides them quarterly). I had to submit the same paperwork again to get the rest transferred, which will hopefully complete by the 12th.
For anyone still trying to transfer before the deadline at the end of the 17th (the "absolute last day to transact" according to Vanguard), I'd recommend escalating on Fidelity's end and making sure they know to fax the request. The fidelity rep got the fax number from the Vanguard small business team at 1-800-992-7188. I don't actually know the fax number myself, but presumably they'd give it to you if you asked.
