I think I may have found the answer....at least I'm very close.

If I add all the "total dividends and distributions" in the detail section, subtract all the "long-term cap gains", do not subtract "short-term cap gains" and ignore "total foreign tax withheld", the total is .07 (7 cents) smaller than "total ordinary dividends" (1a) in the summary.
The foreign tax is included in the total dividends on the 1099-Div AND the separate box for foreign tax
